#1: Food
- XXXXX calories per day
- If fortnightly weight gain goals are not met, calories will be increased by 100 per day (and adjusted if necessary the following fortnight)
#2: Weight Gain
- A weekly average of 0.5kg is expected
- Weigh-ins are fortnightly at CAMHS (may be adjusted at a later date)
#3: Exercise – agreed (to be reviewed as necessary)
- List here what is permitted per week or per day
Ben is not allowed to carry forward ‘un-used’ exercise to another day.
#4: Rewards – Point system
Food:
- 1 point per day Ben keeps to current calorie total
- 2 points for every day Ben exceeds current calorie total by a min of 200
- Challenges: 1 point (resist exercise, challenging meal, eating in between meals)
- Challenges: 2 points (fear foods, eat out successfully, going out with friends, cinema on non-exercise day)
- 3 points deducted for dishonesty
School:
- 1 point for every part morning or full afternoon
- 2 points for every full morning
- 3 points for every full day
Exercise:
- 1 point per day Ben keeps to contract
- 3 points for any non-exercise day
- 3 points deducted for dishonesty
- 2 points deducted when DON’T have 200 calories on extra exercise days
Weight gain:
- 20 points for every fortnightly weigh-in session Ben incs by 1kg
- 18 points for every fortnightly weigh-in session Ben incs by 0.9kg
- 17 points for every fortnightly weigh-in session Ben incs by 0.8kg
- 16 points for every fortnightly weigh-in session Ben incs by 0.7kg
- Less than 0.7kg = zero points
Redeem points at the end of every month

#5: Aim
- To keep the momentum going and avoid ‘going round in circles’
- To see sustained weight gain and marked improvement
- To maintain weight once Weight Restored (WR)
- To eliminate calorie counting and all ‘numbers’
- To maintain weight at WR for 6 months from Christmas to qualify for 2012 University entry
- To agree that no compromise will be made on that level
- To agree to University Transition Contract prior to leaving for Uni
#6: To avoid…
- Being able to claim rewards without achieving sustained weight gain
- Not being WR by 18th birthday at the latest to qualify for 2012 Uni entry
